PPE (PERSONAL PROTECTIVE EQUIPMENT)

In this video we will talk about Personal Protective Equipment or PPE and how to wear it and what is it.

The purpose of using PPE is to minimize occupational hazard. PPE is a part of universal precautions and it is an approach of infection control in which it is said that you have to take the blood and body fluid of every patient as infectious.

Today I will tell you what I use to do.

PPE contains these six things –

  1. Hand sanitizer

  2. Gloves

  3. Head cap

  4. Eye cover

  5. Face cover

  6. Gown

You can do the selection of PPE, keeping in mind that how much you will get in contact with the patient’s body fluid.

For ex – There is less risk in denture patient in comparison to scaling because blood, saliva, water can come to your face.

Let’s see how to wear PPE.

At first you have to keep all the things on a clean place. I have made clothes for these things only. If you don’t have head cap and mask in your kit then you can wear these also.

FIRST STEP IS GOOD HAND HYGINE.

  • Hand washing will take the time of approx one minute.

  • You have to count 1-10 in every step of hand washing.

  • Fold your shirt to the elbow before hand washing

  • Remove your watch, ring etc if you have worn it.

  • The length of nails should be shorter than finger tip.

  • If you have put on nail polish then it should be intact.

  • If your hand is visibly dirty then before using soap wash your hand nicely with running water.

  • Then apply enough soap to cover all the surfaces of hand.

  • You don’t have to close the tap otherwise you will have to touch the tap again.

  • Rub hand palm to palm to obtain a good quality and quantity of foam.

  • Then rub the back of the left hand by the palm of right hand by putting fingers into fingers and vice versa.

  • Then rub palm to palm with finger interlaced.

  • Next, you have to rub back of fingers by opposing palm with finger interlaced.

  • Repeat this action for other hand also.

  • Then hold the left hand with the right palm and rotate it and vice versa.

  • To clean the tips of the finger, rub rotationally backwards and forwards with clasped fingers of right hand in left palm and vice versa.

  • Use soap to wash till your elbow.

  • Rinse hand thoroughly with running water.

  • Dry hand thoroughly with single use hand towel.

  • If you are unable to close the tap from elbow then use this towel to close the tap.

  • You don’t have to touch the tap from your hand.

  • Now, your hand is clean and safe.

Now we are ready to wear rest of the things.

  • Arm holes are given in most of the good kits to grasp the gown.

  • If not then hold the gown from shoulder and let the gown be unfolded.

  • Put the gown collar near your neck and put your arms into the sleeves.

  • You don’t have to put out your fingers from gown cuff.

  • Then your assistant will tie it from your back of the neck.

  • You can tie it from near your waist.

  • Your assistant can walk behind you by holding your tie. Good kit has a tag to hold the assistant but if it’s not there in this then you have to it from your own.

Then comes to wear the gloves.

  • You have to wear the gloves keeping your hands inside.

  • Open it by holding the wrapper

  • You have to hold the wrappers from edges so that you can see the gloves.

  • The inside surface of the wrapper is sterile.

  • Pick any one of the gloves from the folded surface.

  • Keep it in mind do not touch the outer surface of the gloves because the outer surface of the gloves is sterile.

  • Now put your second hand in the gloves.

  • You have to hold the gloves from the inside surface only.

  • Now after wearing the gloves hold the rolled cuff from inside.

  • Now you can do the adjustment of the rest of the gloves.

  • Now throw the wrapper of the gloves by holding it from inside.

  • Now don’t touch anything which is not sterile.

Next if you have not wear head cap from beforehand then ask your assistant to help you. 

  • And if you are alone or you are assisting the surgeon then wear gown first, then head cap, then mask, then hand sanitize, then use gloves.

  • Head cap is to be worn at first.

  • While wearing the head cap, keep it in mind that your all hair should be inside the head cap, especially for girls.

  • You can keep your head cap that much big in which all your hair can be adjusted.

NEXT COMES FACE MASK OR RESPIRATOR

  • Now here basically we have to choose between two things – blue disposal regular face mask or N95.

  • You have to use face mask in every case because in our profession air droplets can come to our face.

  • You have to use N95 mask when you have the doubt of air borne disease like this mask will save you from droplet nuclei from tuberculosis patient.

  • Now place your face mask.

  • Press your mask near the nose so that you can make a seal near the nose.

  • Then take the straps behind the ears.

  • Then cover your chin from the lower part of your mask.

IF YOU ARE USING RESPIRATOR THEN –

  • Follow the instructions of the manufacturer.

  • And you have to check seal after wearing.

  • Take your mask and place it over nose and mouth.

  • Then adapt your nose piece around your nose.

  • After that fix the top strap to the top part of the head and lower strap to the lower part of the head which is near to your neck.

  • Then you have to do seal check.

  • Put your hand on the mask, then breathe comfortably and feel the air leakage.

NEXT STEP IS EYE PROTECTOR

  • You can use Goggle.

  • Eye shields also come.

  • There is no side protection in spex so if you don’t use spex then use eye protection from which side protection could be done.

  • Eye protector should be fit. It should not be tight enough.

  • Now you are ready to perform your task.

  • This should be kept in mind that you should not touch any non-sterile object.

 

Now let’s see how to remove PPE.

Order of removing PPE (Dotting)

  • Gloves

  • Goggle/face shield/eye protector

  • Gown

  • Mask/respirator

  • Head cap

  • Perform hand hygiene

Before removing these all you should keep in mind that which objects should be refer as clean and which should be refer as non-clean.

The contaminated surface of the gloves is outer surface.

The front part and sleeves of the gown is contaminated.

Your spex or protector is also contaminated.

The front surfaces of mask or respirator, these all things are contaminated.

At first you have to remove gloves.

Hold the cuff from one hand and remove it.

Hold that gloves from the second hand.

Rotate your finger inside your gloves and remove the gloves of the next hand.

Then remove your spex, goggle and eye-protector.

Hold these from back near the ear.

Because I use spex so my assistant used to clean it with soap and water and then after cleaning it from surface disinfectant, he gives it to me.

Then remove the gown. Remember you don’t have to touch the front surface of the gown.

Untie waist and neck ties.

Hold gown from inside and remove it.

Then we will remove our mask but remember we have to hold it from strap only.

You don’t have to remove the respirator until you don’t make your patient free.

So if you have to give instruction to your patient first wash your hand and then give instructions.

You have to remove respirator after discharging the patient.

At first you have to remove the lower strap of the respirator then the upper strap respectively.

You don’t have to touch the respirator.

Remove head cap.

And at last perform hand hygiene.
